Safe and effective home treatment of infections

Certain infections, such as those affecting bones, joints, heart valves, or the prostate, require long-term intravenous (IV) antibiotics. With OPAT (Outpatient Parenteral Antimicrobial Therapy) integrated into Remecare’s care pathway, these treatments can be safely and effectively continued at home, allowing patients to return to their daily routines quickly.

Remecare digitizes the treatment plan and actively engages all healthcare providers, including home nurses, general practitioners, specialists, and hospital pharmacists, each fulfilling their specific roles. Remecare's technology ensures the OPAT program is efficiently organized, offering you, the treating physician, the following benefits:

Streamlined discharge & initiation of OPAT patients

Discharging an OPAT patient involves significant administrative work, and primary care providers must be promptly informed. Remecare simplifies these tasks and seamlessly integrates into your existing EHR software.

Initiation process

Remecare ensures that all necessary information reaches the appropriate stakeholders upon patient discharge. The initiation process includes therapy-specific questions (such as required medications) and allows the addition of necessary documents (such as prescriptions).

Engage the entire care team

When patients are discharged from the hospital, they are assigned a team of healthcare providers. Depending on their role (general practitioner, home nurse, hospital pharmacist, etc.), each stakeholder receives different information regarding the patient's treatment. This ensures seamless communication with colleagues in primary care.

EHR integration

The treating physician prefers to initiate the OPAT care pathway directly from their own EHR software package. Thanks to the Circle-of-Trust (COT) integration, this can be done from various EHR systems. Patient data is automatically transferred to the care pathway in Remecare, and the prescriber's information is shared. This process is quick and helps prevent errors.

Extend hospital-quality care to the patient's home

As a specialist physician, you remain responsible for overseeing an OPAT patient when they are outside the hospital. Remecare digitizes the care plan, ensuring the continuation of high-quality care at home. Additionally, you receive only actions and information relevant to your role.

Medical Orchestrator

Remecare acts as a medical orchestrator, ensuring that all stakeholders are guided and receive the right actions at the right time. This is particularly important in complex care pathways like OPAT, where multiple stakeholders are involved, and timing between their actions is critical.

Role-based actions & information

Remecare generates actions based on roles and ensures that each stakeholder only receives the information relevant to them. For example, in an OPAT treatment, home nurses will need to perform specific actions that apply only to them, such as completing the necessary training on the type of antibiotic and how to administer it. This way, an IV antibiotic treatment can be continued at home with the same quality of care as in the hospital.

Secondary use of health data

Remecare easily exchanges data with different systems via the FHIR protocol. An FHIR server makes all relevant medical data available for secondary use. This means that health data can be aggregated at a population level with the aim of gaining and/or supporting new and meaningful insights to further improve the quality of care provided, benefiting patients, healthcare providers, and healthcare systems.
